Job Summary

We are seeking a highly motivated and compassionate Early Intervention Specialist to join our team at Southeastern Michigan Health Association. As a key member of our community health team, you will play a vital role in providing support and resources to individuals living with HIV.

Key Responsibilities
  • Provide on-site and on-call program orientation to clients in various settings, including emergency departments, medical clinics, and testing sites.
  • Determine client-identified needs through a strength-based assessment and develop a care plan with the client.
  • Establish and maintain up-to-date client records, including demographics, proof of status, progress notes, and signed consents.
  • Assist in medical insurance enrollment, scheduling medical appointments, and attending with clients as needed.
  • Develop and maintain a network referral system in the healthcare and social service community.
  • Discharge and connect clients to follow-up services, such as medical case management.
  • Collect and submit data reports at designated intervals for program monitoring and evaluation.
Requirements
  • Strong interpersonal skills and ability to collaborate with clients, colleagues, and community partners.
  • Exceptional organizational skills and strong problem-solving skills.
  • Excellent written and verbal communication skills.
  • Proficient in Microsoft Office or related software programs.
Preferred Qualifications
  • Knowledge of HIV disease.
  • 3 years' experience working with persons living with HIV.
  • Current Michigan driver's license, insurance, and reliable personal transportation.
