Job Description

We are seeking an experienced Case Manager to join our team at Penndel Mental Health Center. As a key member of our staff, you will work directly with individuals diagnosed with mental illness, intellectual disabilities, or drug and alcohol addiction.

Key Responsibilities:

  • Provide face-to-face contact with clients and their families in the community to engage and advocate for their needs.
  • Develop individual service plans to help clients access resources and services such as mental health services, housing, and medical care.
  • Coordinate services with various providers, including mental health programs, hospitals, and residential rehabilitation facilities.
  • Assist clients during crisis situations and provide support to resolve these situations.

Requirements:

  • Bachelor's degree in a related field such as sociology, psychology, or social welfare.
  • Valid driver's license and ability to drive a compact company vehicle.
