Job Summary

This role offers an exciting opportunity to make a real difference in the lives of individuals with mental health challenges. As a Behavioral Health Case Manager, you will play a critical role in ensuring that our clients receive comprehensive support services and navigate complex healthcare systems.

Your responsibilities will include developing and implementing individualized service plans, coordinating with healthcare providers, and advocating for our clients' needs. You will also have the opportunity to work closely with other team members to ensure seamless care coordination and delivery.

Key Responsibilities:

  • Develop and implement individual service plans that address consumer strengths and goals.
  • Coordinate services provisions from various providers and participate in treatment planning meetings.
  • Provide face-to-face contact and support to individuals and families in the community.
  • Advocate for consumers to access essential services and supports.
