Job Summary:

As a Case Manager with The MetroHealth System, you will play a vital role in ensuring the best possible outcomes for our patients. You will interact with third-party payers, external case managers, employers, post-acute service providers, and the interdisciplinary team to negotiate benefits and care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Assess patients for potential case management intervention
  • Negotiate benefits and care plans between patient, team, family, and payer
  • Monitor the effectiveness of the care plan and customer/third-party payer satisfaction

Requirements:

  • Bachelor's Degree in Nursing (BSN) from an accredited institution
  • Current Registered Nurse (RN) licensee for the State of Ohio
  • Three years of hospital in-patient care or home care experience
  • Able to work independently and as a member of interdisciplinary teams
  • Effective interdisciplinary, verbal, and written communication skills

Preferred Qualifications:

  • Master's Degree in Nursing (MSN) from an accredited institution
  • Certification in Case Management from a recognized issuing body
