Job Description

Job Summary: We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Revisit LPN at Hartford HealthCare at Home. As a Revisit LPN, you will play a vital role in providing personalized care to our patients in their homes.

Key Responsibilities:

  • Facilitate communication between the patient and the interdisciplinary team to ensure seamless care coordination.
  • Develop and maintain a therapeutic relationship with patients and their families by providing education, promoting self-advocacy, and independence.
  • Utilize state-of-the-art technology to document skilled services provided, ensuring accurate and timely reporting.

Requirements:

  • Licensed Practical Nurse with a valid CT LPN license.
  • One year of LPN experience, with homecare experience preferred.
  • Valid driver's license and reliable transportation required.
  • Computer proficiency required.
