Responsibilities- Provide direct patient care, including administering medications, wound care, and other medical interventions.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with the interdisciplinary care team to ensure seamless care coordination.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Certified Nursing Assistant (CNA) license in the state of Minnesota.
- Valid CPR certification.
- High school diploma or equivalent.
- Ability to travel to all business locations.
- Meet the regulations and requirements of the state(s) in which the program provides service.
