We are in search of a skilled Physical Rehabilitation Specialist to establish and nurture relationships within the community, promoting our services and aiding in the growth of our Medicare home health initiatives in the Worcester Metro area.

The ideal candidate will be self-motivated, results-oriented, and adept at building enduring relationships.


JOB OVERVIEW:
A Licensed Physical Rehabilitation Specialist (PT) delivers physical therapy services to patients intermittently in their homes.

This is conducted in accordance with physician directives and care plans under the supervision of the Director of Clinical Services/Clinical Manager.


QUALIFICATIONS:


Graduation from a physical therapist program accredited by the Commission on Accreditation in Physical Therapy Education or equivalent bodies.
Current licensure in the state(s) of practice.
Preferred: Two (2) years of relevant experience.
Alignment with the philosophy and objectives of our agency.
Ability to demonstrate initiative and independent judgment.


KEY RESPONSIBILITIES:
Adhere to established policies and procedures.
Administer physician-ordered physical therapy.
Enhance or alleviate physical disabilities of patients.
Assist individuals in achieving their optimal functional levels.
Collaborate with other home care professionals in patient care planning.
Supervise staff as necessary.
Conduct initial assessments, develop care plans, and perform regular reassessments.
Execute all skilled procedures as prescribed by physicians.
Consult with physicians regarding treatment modifications.
Document patient progress and communicate with physicians.
Educate patients and their families on home care programs and daily living activities.
Engage in in-service training and contribute to quality improvement initiatives.
Attend scheduled patient care meetings.
Prepare clinical documentation and progress notes.
Assist physicians in evaluating patient functionality.
Contribute to the development and revision of care plans.
Engage with families and agency personnel.

Complete and submit OASIS assessments and related documentation in accordance with agency timelines.

Utilize ICD-10 codes appropriately.

WORK ENVIRONMENT:
Work primarily indoors in agency offices and patient residences, with travel required between locations.

Director of Clinical Services/Clinical Manager

SUPERVISES:
Physical Therapy Assistants and Home Health Aides as assigned
