Licensed Practical Nurse for Assisted Living Facility

Key Responsibilities:
- Assist in the development of resident care and service plans by gathering relevant data based on individual age, needs, and capabilities.
- Facilitate effective communication with residents, their families, and the healthcare team.
- Provide education, guidance, and support to residents and their families.
- Ensure accurate documentation in the client service record.
- Conduct supervisory assessments of resident care assistants.
- Engage in educational initiatives and apply current literature to improve professional skills.
- Manage medication administration in collaboration with the healthcare team.
Minimum Qualifications:
- Education: Graduate from an accredited School of Nursing.
- Experience: 1-2 years of clinical experience is preferred.
- Licenses: Valid LPN license and CPR certification.
