Hospice Home Health Nurse Job Summary

We are seeking an experienced Home Health Nurse to join our Hospice team. As a member of our team, you will provide direct patient care in the comfort of their own homes, developing and implementing individualized care plans and collaborating with interdisciplinary teams to ensure high-quality patient outcomes.

Key Responsibilities
  • Conduct initial assessments and develop care plans for patients
  • Provide skilled nursing care, including wound management and medication education
  • Collaborate with interdisciplinary teams to coordinate patient care
Salary and Benefits

Requirements
  • Graduate of a school of nursing
  • Washington Registered Nurse License upon hire
  • Provider BLS - American Heart Association within 30 days of hire
