We are seeking a skilled and compassionate Registered Nurse to join our team at Family Service Association's Adult Day Health program in Mashpee, MA.
Job Summary:The Registered Nurse will be responsible for ensuring the proper medical care of participants, including acting as liaison between participants' primary care physicians, monitoring participants' health status, and supervising program staff.
Responsibilities:- Serve as liaison between physician and participant for medical information and service authorization of participants.
- Coordinate the development, on-going review, and revisions of each participant's plan of care.
- Monitor participants' health status, and respond to any acute changes in the participants' health or functioning.
- Administer medication and document on medication sheets.
- Complete participant daily flow sheets and record monthly vitals.
- Supervise other healthcare staff members.
- Provide case management services.
- Review and supervise participants' personal care plans at least bi-annually or quarterly.
- Attend supervisory meetings at least monthly.
- Supervise program aides and all other services involving health issues of participants.
- Complete nursing progress notes in each participant's record at least monthly.
- Assist the Program Coordinator with the implementation of policies and procedures.
- Assume the role of Assistant Program Coordinator in the Program Coordinator's absence.
- Ensure on-going education, training, and professional development of all program staff relevant to the care of participants.
- Licensed by the Board of Registration to practice in the Commonwealth of Massachusetts and in good standing.
- At least two (2) years recent experience in the direct care of older adults or adults with disabilities.
- CPR and First Aid Certified.
