Position Overview
The Patient Care Assistant is tasked with delivering essential medical support and aiding with daily living tasks that enhance the well-being of our clients, enabling them to maintain their independence in a home or facility setting while upholding confidentiality and professional boundaries at all times.

Key Responsibilities
- Provide, assist, assess, and document medical care, including:
  • Basic first aid
  • Monitoring weight and vital signs
  • Catheter and bowel management, including intake and output tracking
  • Range of motion exercises and other activities as directed by rehabilitation professionals
  • Assisting with mobility, transfers, and positioning using appropriate medical equipment
  • Meal preparation tailored to dietary needs
  • Addressing specific patient conditions within the authorized scope of practice

- Support clients in maintaining personal hygiene by:
  • Assisting with bathing, grooming, and dressing
  • Providing oral hygiene and skin care
  • Helping with toileting and feeding

- Ensure thorough documentation and coordination of care, including:
  • Completing necessary patient and family records after each visit
  • Reporting significant changes in the patient's condition to the supervising nurse

Qualifications
- High school diploma or equivalent
- Valid certification or proof of training as mandated by state regulations
- Current CPR certification
- Proven ability in clinical assessments and adherence to care policies
- Valid driver's license and acceptable driving record

Additional State-Specific Requirements
Continuing education as required by state regulations.

Physical Requirements
- Proficient in speaking, writing, reading, and understanding English
