About the Role

We are seeking a highly skilled Transitional Care Provider to join our team at Complete Health. As a key member of our transitional care team, you will work closely with patients, families, and healthcare providers to ensure seamless transitions of care.

Key Responsibilities:
  • Develop and implement individualized care plans for patients with complex health needs.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed.
  • Collaborate with interdisciplinary teams to coordinate secondary services and ensure continuity of care.
  • Provide education and support to patients and families to promote self-care and independence.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • National certification as a Nurse Practitioner.
  • Current DEA license.
  • Master's degree in Nursing from an accredited program.
  • Minimum 3-5 years of clinical experience, preferably in EMT or Paramedic background.
  • 1-2 years of Nurse Practitioner experience, preferably in inpatient hospital or skilled nursing settings.

Estimated Salary: $180,000 - $250,000 per year.
