Job Summary

We are seeking a skilled Care Coordinator to join our interdisciplinary team at UTMB Health. As a key member of our team, you will be responsible for planning, coordinating, and safely transitioning acute care patients to the next level of care.

Responsibilities
  • Partner with physicians, nursing, and ancillary services to ensure quality outcomes and coordinate patient care.
  • Coordinate the care of patients and ensure timely utilization of services.
  • Develop and implement safe and timely transition plans.
Requirements
  • RN with Bachelor's degree in nursing.
  • Four years of clinical nursing experience in an acute hospital environment.
  • Current license or valid permit to practice professional nursing in Texas.
  • Certification in Care Management CCM or ACM-RN obtained within 3 years.
