Responsibilities
  • Provide comprehensive care and services to a group of patients, including psych-social assessments, interventions, and knowledge of community resources.
  • Work collaboratively with nurses, social workers, chaplains, volunteers, and CNA leadership to ensure seamless patient care.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare professionals to ensure informed decision-making and optimal patient outcomes.
  • Manage multiple priorities and tasks, including documentation, meetings, and travel, to ensure efficient and effective care delivery.
Requirements
  • Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Strong time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to learn and utilize electronic medical records systems.
  • Reliable transportation, valid driver's license, and auto insurance, to travel across the service area.
