Suncrest Hospice is seeking a dedicated and compassionate Registered Nurse (RN) to join our team. As a key member of our interdisciplinary team, you will provide exceptional care and support to patients and their families in their homes, skilled nursing facilities, and assisted living facilities.
Responsibilities- Provide comprehensive care and services to a group of patients, including psych-social assessments, interventions, and knowledge of community resources.
- Work collaboratively with nurses, social workers, chaplains, volunteers, and CNA leadership to ensure seamless patient care.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Communicate effectively with patients, families, and healthcare professionals to ensure informed decision-making and optimal patient outcomes.
- Manage multiple priorities and tasks, including documentation, meetings, and travel, to ensure efficient and effective care delivery.
- Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Strong time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Ability to learn and utilize electronic medical records systems.
- Reliable transportation, valid driver's license, and auto insurance, to travel across the service area.
