Key Responsibilities:

  • Conduct thorough assessments through interactions with patients and healthcare professionals to gather and analyze psychosocial data, identifying discharge needs.
  • Develop and implement treatment plans that address patient acuity and management issues, ensuring that goals are measurable and achievable.
  • Maintain accurate and timely documentation for individual, family, and group therapy sessions.
  • Provide individualized therapy, meeting with patients regularly to evaluate their progress and adjust treatment goals as necessary.
  • Communicate effectively with the treatment team regarding patient progress and readiness for discharge, ensuring all plans are documented within required timelines.
  • Regularly update service plans to reflect the evolving needs of participants, securing necessary consents.
  • Adhere to documentation protocols, submitting all required notes and logs promptly.
  • Perform additional duties as assigned to support the mission of JEMCARE LLC.

Qualifications:

  • A minimum of three years of experience working with medically fragile populations, with at least one year of clinical experience in pediatric care, particularly in end-of-life contexts.
  • Possession of a valid license as a Clinical Social Worker (LCSW), Master Social Worker (LMSW), Psychologist, Mental Health Counselor (LMHC), or Creative Arts Therapist (LCAT) in accordance with New York State guidelines.
  • Student interns may participate under supervision, adhering to New York State Education law.
