Job Summary:

Responsibilities:

  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between departments.
  • Provide education and support to patients and their families, promoting health literacy and self-management skills.
  • Monitor and report patient progress, identifying areas for improvement and implementing evidence-based interventions.
  • Supervise and mentor junior staff members, promoting a culture of excellence and continuous learning.
Requirements:
  • Current RN licensure in the state of New York.
  • 2+ years of experience in a healthcare setting, preferably in a similar role.
  • Strong clinical skills and knowledge of nursing practices and procedures.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
