We are seeking a skilled Medical Care Coordinator to join our team at Universal Health Services. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring seamless coordination of medical services.

Job Summary:

The Medical Care Coordinator will work closely with our Pharmacist and Program Physician to assist with medication administration and coordination of medical care. This includes conducting nursing evaluations, monitoring client status, and preparing medications for distribution.

Responsibilities:
  • Conduct comprehensive nursing evaluations to identify patient needs and develop personalized care plans.
  • Maintain accurate records of patient medication usage and monitor for any adverse reactions or interactions.
  • Collaborate with the healthcare team to ensure timely and effective delivery of medical services.
  • Provide ongoing education and support to patients and their families on health-related topics.
  • Develop and maintain relationships with community resources to facilitate referrals and coordinate care.
  • Participate in staff meetings and treatment team discussions to share knowledge and best practices.
