We are seeking a highly skilled and experienced Social Services Supervisor II to lead our team of social workers and case managers. In this role, you will be responsible for overseeing the clinical and case management functions and providing administrative and programmatic supervision to case management staff. Key responsibilities include:
• Supervising daily operations and clinical staff in the absence of the Director of Social Services/Program Director
• Providing administrative and programmatic supervision to case management staff
• Supporting and guiding case managers in the execution of all housing-related tasks
• Meeting individually with direct reports for weekly supervision
• Assigning cases to case managers and scheduling work according to priority
• Conducting case conferences, reviewing cases, and resolving client issues
• Auditing client records and ensuring compliance with Urban Pathways, regulatory, and funding agency requirements and guidelines
• Assessing, training, and coaching case managers on work assignments and community resources
• Supervising and documenting incident reporting and distributing related notices within 24 hours
• Monitoring client medications, as required
• Reviewing service plans, program notes, and other required client documentation to ensure comprehensiveness and compliance with Urban Pathways and regulatory agency requirements
• Ensuring case managers are adhering to the monthly 360 Chart Audit
• Ensuring housing documentation is completed in a timely manner for submission to housing providers
• Managing caseload in the absence of the Case Manager
• Completing and submitting required reports and statistics in a timely manner
• Preparing staff schedules and monitoring payroll
• Supervising client databases (AWARDS, CARES, etc.) to ensure accuracy of client information
• Developing and maintaining relationships with community services and resources
• Facilitating staff meetings, trainings, and supervisory sessions, as required
• Being on call to address emergency needs on a 24-hour basis

The estimated salary for this position is $75,000-$85,000 per year.
