About the Role

We are seeking a highly skilled and experienced RN to join our team as an Inpatient Case Manager. As a key member of our healthcare team, you will play a critical role in optimizing quality and efficiency of care for hospitalized members.

Key Responsibilities
  • Collaborate with physician partners to develop and implement individualized plans of care to ensure continuity, quality, and appropriate resource use.
  • Review and monitor patients' hospital stays to ensure timely and efficient delivery of essential services.
  • Develop and coordinate discharge plans to meet each patient's identified needs, communicating with physicians, patients, families, and community agencies as needed.
  • Perform psychosocial assessments on high-risk patients and provide education to assist with discharge and coping with psychological problems related to acute and chronic illness.
  • Document all admissions and discharges in the patient's electronic medical record and make post-discharge follow-up calls to patients not referred to an ambulatory case/care management program.
Requirements
  • Minimum two years of experience in patient care delivery or completion of a Master's degree in Case Management.
  • Associates Degree in Nursing.
  • Licensed Registered Nurse (Temporary - Georgia).
  • Demonstrated advanced communication and interpersonal skills with all levels of internal and external customers.
  • Ability to collaborate effectively with a multidisciplinary healthcare team.
  • Functional knowledge of computers and experience with managed healthcare delivery, including Medicare.
