RN, Inpatient Case Manager, Full Time

We are seeking a skilled RN to join our team as an Inpatient Case Manager. This is a full-time position that requires collaboration with physician partners to optimize quality and efficiency of care for hospitalized members. The successful candidate will be responsible for carrying out daily utilization and quality review, monitoring for inefficiencies and opportunities to improve care, and developing a safe discharge plan to include recommending alternative levels and sites of care when appropriate.
Key Responsibilities:
- Plan, develop, assess, and evaluate care provided to members, collaborating with physicians and other healthcare team members to ensure continuity, quality, and appropriate resource use.
- Review and monitor patients' hospital stays to ensure timely and efficient delivery of essential services, communicating with hospitalist partners through regular huddles.
- Begin discharge planning immediately for new inpatient admissions, assessing high-risk patients in need of post-hospital care planning, and developing and coordinating the implementation of a discharge plan to meet each patient's identified needs.
- Perform psychosocial assessments on high-risk patients, comprehensively assessing their goals, biophysical, psychosocial, environmental, economic/financial, and discharge planning needs, and providing education to assist with discharge and help patients cope with psychological problems related to acute and chronic illness.
- Document all admissions and discharges in the patient's electronic medical record, making post-discharge follow-up calls to patients not referred to an ambulatory case/care management program.
- Act as a liaison between inpatient facilities and referral agencies, providing case management to patients referred and serving as an advocate for patients and families, coordinating transfers to appropriate facilities and maintaining required documentation.
Requirements:
- Minimum two years of experience in patient care delivery or completion of a Master's degree in Case Management in lieu of minimum years of experience.
- Associates Degree in Nursing.
- Registered Professional Nurse License (Georgia) or Licensed Registered Nurse (Temporary - Georgia).
Preferred Qualifications:
- Minimum five years of clinical nursing experience in a hospital setting.
- Minimum five years of professional practice experience in an acute care setting.
- Minimum two years of experience in utilization review, case management, and discharge planning.
- Bachelor's Degree in Nursing, Health Care, or Master's degree in Case Management.
