Responsibilities- Provide direct patient care, including administering medications, monitoring vital signs, and documenting health concerns
- Collaborate with healthcare teams to develop and implement individualized care plans
- Conduct intakes, document health concerns, and monitor medical history of residents
- Administer and properly verify medications, monitoring their effects
- Review residents' charts and medical records, ensuring accurate and up-to-date information
- Assist with the cultivation of a safe, comfortable facility, maintaining appropriate stocks of medical supplies
- Participate in IV set up, immunizations, and phlebotomy procedures, as needed
- Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
- Completion of approved Licensed Practical Nurse (LPN) educational program
- High school diploma (or GED/equivalent)
- Minimum of 6 months of experience (LPN or related role)
- Ability to lift a minimum of 50 lbs
