Family Support Specialist

At NashvilleHR, we're seeking a compassionate and dedicated Family Support Specialist to join our team. As a Family Support Specialist, you'll play a vital role in supporting families impacted by behavioral health challenges and connecting them with vital resources.
Key Responsibilities Include:
- Family Engagement:
- Build trusting relationships with families and caregivers through regular communication and home visits.
- Conduct needs assessments to identify family strengths, challenges, and goals.
- Develop and implement individualized family support plans in collaboration with families and the behavioral health team.
- Resource Coordination:
- Connect families with community resources, services, and support networks.
- Assist families in navigating systems such as healthcare, education, and social services.
- Provide information and guidance on accessing financial assistance, housing, and other essential services.
- Advocacy and Support:
- Advocate for the needs and rights of families and children within various systems and settings.
- Offer emotional support, guidance, and practical assistance to families in crisis.
- Facilitate support groups and workshops for parents and caregivers to enhance their knowledge and skills.
- Collaboration and Teamwork:
- Work closely with clinicians, case managers, and other professionals as needed to support family goals.
- Participate in multidisciplinary team meetings to discuss family progress and coordinate care.
- Maintain accurate and up-to-date documentation of all interactions and services provided.
What We're Looking For:
- TDMHSAS Family Support Specialist Certification (required)
- 2+ years of experience in a behavioral health or social services setting
- Bachelor's degree in social work, psychology, counseling, or related field (preferred)
