We are seeking a compassionate and dedicated Home Health Care Provider to join our team at St. Joseph's Home Health, Inc. As a Home Health Care Provider, you will play a vital role in providing high-quality care to our elderly patients in the comfort of their own homes.
Responsibilities
- Provide personal care and hygiene assistance to patients, including bathing, dressing, and grooming.
- Assist patients with daily living activities, such as eating, toileting, and mobility.
- Remind patients to take their prescribed medication and assist with medication management.
- Help patients with light housekeeping and laundry duties.
- Prepare and serve meals to patients, as needed.
- Accompany patients to appointments and outings, as needed.
- Monitor patients' condition and report any changes or concerns to the healthcare team.
- Provide emotional support and companionship to patients.
Requirements
- High School Diploma or equivalent required.
- Minimum 1 year of experience in a healthcare, hospitality, or in-home care environment.
- Valid driver's license and reliable transportation required.
- Ability to lift and transfer patients, as needed.
- Ability to work varying shifts, including nights and weekends.
- Ability to work independently and as part of a team.
- Validated ability to act in a compassionate and supportive manner.
