Hospice LPN Weekend Caregiver

We are seeking a compassionate and skilled LPN to join our Residential Home Health and Hospice team. As a Hospice LPN, you will collaborate with our extraordinary care team to provide management and delivery of patient care plans.

This position supports patients in Midland, MI and surrounding areas. The schedule is every Friday - Sunday, 8am - 8pm.

We are looking for compassionate nurses with:
- LPN license in the state you work
- Graduate from an approved school of practical nursing
- Two years of LPN experience, hospice preferred
- Current driver's license and ability to spend ~20% of your day driving to/from patient locations
- A commitment to consistently meet critical deadlines for charting
- The skills needed to self-manage your time and schedule
- Demonstrated experience with tablets, mobile phones and EMR software
