Key Responsibilities:

  • Deliver high-quality medical care, including thorough evaluations, accurate diagnoses, and effective management of medical conditions.
  • Conduct detailed medical histories and physical examinations, while also ordering and interpreting necessary diagnostic tests.
  • Formulate treatment plans, write prescriptions, and ensure ongoing follow-up for optimal health outcomes.
  • Maintain accurate and compliant documentation of medical care in participant records, adhering to established policies.
  • Participate in after-hours on-call coverage, triaging calls with the support of nursing staff.
  • Collaborate with an interdisciplinary team to develop and implement individualized care plans for participants.
  • Engage in quality improvement initiatives to enhance the effectiveness of our healthcare programs.
  • Lead and motivate team members to foster a positive and collaborative work environment.
  • Perform additional duties as assigned to support the organization’s mission.

Qualifications:

Required:

  • Doctor of Medicine or Osteopathy degree.
  • Active state Medical License and DEA license with full prescriptive authority.
  • Board Certification in a primary care specialty or Board Eligible with certification required within the first year of employment.
  • A minimum of one year of experience working with elderly or frail populations.
  • Not listed on the CMS Preclusion List.

Preferred:

  • Postgraduate training in a primary care specialty, such as internal or family medicine.
  • Completion of post-residency training in geriatric medicine.
  • Bilingual capabilities.
