About the Role

As an integral part of our comprehensive Geriatrics team, you will play a key role in reducing unnecessary ER visits and hospitalizations by supporting primary care providers and delivering high-quality, compassionate care. You will have the opportunity to work with a complete interdisciplinary team, including geriatricians, social workers, pharmacy, and case management, to provide comprehensive care to our patients.

Responsibilities

Provide personalized chronic disease management for complex elderly patients in their homes, assisted living facilities, and board and care settings.
Work collaboratively with an interdisciplinary team to ensure the best outcomes for our aging population.
Support primary care providers in reducing unnecessary ER visits and hospitalizations.

Qualifications

Valid and unrestricted license to practice in the State of California.
Nationally recognized certification (as determined by Medicare).
Completion of state-mandated educational courses.
