Home Health Speech Language Pathologist

We are seeking a skilled Speech Therapist to join our Home Health team at National Church Residences. As a Speech Therapist, you will play a vital role in evaluating and treating residents with communication and swallowing disorders, enabling them to develop, recover, and improve the skills needed for daily living and working.
Key Responsibilities- Evaluate and treat residents with communication and swallowing disorders through the therapeutic use of everyday activities.
- Develop and evaluate plans of care in partnership with patients, patient representatives, and caregivers.
- Provide patient and caregiver education and counseling.
- Communicate timely with physicians and other healthcare practitioners involved in the plan of care.
- Supervise home health/hospice aides as needed.
- Complete necessary documentation accurately and timely.
- Assist with marketing and program development as requested.
- Maintain progressive knowledge of the speech therapy field.
- Master's Degree in Speech Language Pathology, Speech and Hearing Sciences, or Communication Disorders.
- Two to four years of experience preferred.
