About the Role

As a Medical Case Manager, you will play a critical role in coordinating patient care, developing treatment plans, and collaborating with families to ensure optimal outcomes.

Responsibilities

  • Coordinate patient care across multiple settings, including hospitals, clinics, and community-based programs
  • Develop and implement individualized treatment plans that address the unique needs of each patient
  • Collaborate with families, healthcare providers, and other stakeholders to ensure seamless care coordination
  • Provide education and support to families to empower them to advocate for their child's needs

Requirements and Qualifications

  • Bachelor's degree in social work, nursing, or related field
  • Minimum 2 years of experience in case management or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Salary and Benefits

The salary range for this position is $34.41 - $51.62 per hour, based on experience. We offer a comprehensive benefits package, including medical, dental, vision, 403(b) retirement plan, life insurance, paid time off, and continuing education opportunities.
