Job Summary

Counseling Service of Addison County, Inc. is seeking a highly skilled and experienced Case Management Professional to join our team. As a key member of our staff, you will be responsible for developing, implementing, and monitoring supports for individuals with intellectual disabilities and potentially traumatic brain injuries, as well as the elderly.

Key Responsibilities
  • Develop and implement individualized support plans to promote independence and quality of life
  • Provide case management services to ensure access to necessary resources and services
  • Collaborate with interdisciplinary teams to coordinate care and services
  • Conduct regular assessments and evaluations to monitor progress and make adjustments as needed
Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in a related field, such as social work, psychology, or special education
  • A minimum of 3 years of experience in case management or a related field
  • Strong supervisory, organizational, and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to respond to emergency situations and some after-hour calls
