Key Responsibilities:
- Perform patient needs assessments and facilitate referrals to health, wellness, and post-acute care resources.
- Collaborate with interdisciplinary teams to progress the plan of care and discharge plan.
- Coordinate care and services within the case managed population, ensuring seamless transitions and optimal outcomes.
- Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care.
- Stay up-to-date with regulatory requirements and ensure compliance with CMS regulations.
Requirements:
- Current licensure in Virginia as a Registered Nurse.
- Three years of recent experience in a clinical healthcare setting with responsibilities reflecting direct management of patient care.
- Supervisory or leadership experience is preferred.
