About the Role

Cook Children's Healthcare is seeking a highly skilled and experienced Patient and Family Support Services Manager to join our team. As a key member of our Neighborhood Clinics, this role will play a critical part in ensuring that patients' psychosocial needs are addressed appropriately.

Key Responsibilities
  • Collaborate with Primary Care Providers and healthcare teams to identify and address patients' psychosocial needs.
  • Supervise case management and community engagement efforts performed by family advocates.
  • Act as process owner and subject matter expert for patient and family support programs.
  • Work with various departments to implement and manage population health initiatives that support Cook Children's strategic goals.
Requirements
  • Masters level social work degree and licensure (LMSW or LCSW) required.
  • Five years of social work experience, including working with children and families.
  • Two years of experience working in a healthcare setting.
  • Minimum one year of supervisory experience, including managing the performance of others.
  • Bilingual English and Spanish a plus.

  • Adept at working in a dynamic team environment.
  • Skilled in program development and implementation.
  • Critical thinking, problem-solving, and self-motivated initiative.
  • Ability to plan and organize effectively.
  • Proficient in the use of computer applications.
Certification/Licensure

Masters level Social Work license (LMSW or LCSW) current, unrestricted Texas license required.
