The Community Residences organization is seeking a skilled Care Coordinator for Individuals to join our team. In this role, you will be responsible for coordinating care and services for individuals with developmental disabilities.

Duties and Responsibilities
  • Implement Individual Plans of Care and Follow-Along Plans based on the Interdisciplinary Team's identification of client needs.
  • Record data on identified teaching strategies, behavior plans, and report client progress and any barriers that impede progress.
  • Monitor medical, physical, and behavioral changes in individuals on a daily basis and report changes to the Supervisor, RN Program Coordinator, or Program Director as it occurs.
  • Administer medication to clients at prescribed times, following agency policy and DDS regulatory standards.
  • Plan, purchase, and prepare meals within available budgetary resources and ensure clients' dietary plans are followed.
  • Maintain the appearance, cleanliness, and upkeep of the house, properties, and vehicle.

A strong understanding of human services principles and practices is essential for success in this role.
