About This Role
This position serves as the liaison and communicator between patients, caregivers, healthcare providers, and multi-disciplinary team members. You will discuss alternative care options with patients/caregivers and assist with discharge planning needs. Your goal will be to facilitate smooth transitions along the healthcare continuum, ensuring quality and cost-effective outcomes.

Requirements
The ideal candidate will have a registered nurse diploma and at least 2 years of nursing experience. A current registered nurse (RN) license in the state of practice is required, as well as Basic Life Support certification from the American Heart Association. Preferred qualifications include an associate's or bachelor's degree in nursing and certification in a clinical specialty area.

Essential Functions
- Provide continuity of care by ensuring smooth transitions between care settings
- Develop relationships with patients and their multidisciplinary teams to facilitate and/or navigate through subsequent treatment and follow-up
- Manage high-risk, complex patient care to minimize readmission
- Work closely with physicians to coordinate patient care plans and communicate with the multidisciplinary team
- Prepare, execute, and reinforce post-discharge care plans

Working Conditions
This role requires occasional exertion of up to 100 pounds of force and frequent exposure to blood, body fluids, and tissue. The incumbent works in a patient care area and may be exposed to hazardous medications or hazardous waste within the environment.

Benefits and Salary
We offer competitive salaries and benefits packages, including health insurance, retirement plans, and paid time off. The estimated salary for this role is $80,000 - $110,000 per year, based on location and experience.
