Key Responsibilities:


Work Schedule:
Monday to Friday, 8-hour shifts between 6:00 AM and 6:00 PM
No Weekends or Holidays


Core Responsibilities:
  • Engage in all aspects of the Case Management Program (CMP) to ensure compliance with established standards of care.
  • Provide nursing expertise in the case management process, including assessment, planning, implementation, coordination, and monitoring.
  • Identify and integrate local case management processes and opportunities.
  • Develop and implement strategies utilizing inpatient, outpatient, onsite, and telephonic case management.
  • Create tools to support case management, including patient identification and assessment resources, clinical guidelines, and databases for community resources.
  • Collaborate with community agencies and organizations to enhance patient care.
  • Accurately document patient care data and develop comprehensive treatment plans to ensure continuity of care.
  • Facilitate effective communication with patients and caregivers, addressing identified learning needs.
Additional Duties:

Utilize automated programs and technology to enhance patient management and engagement. Communicate with patients through secure messaging platforms to improve care coordination.

Participate in multidisciplinary team meetings to address complex patient needs and develop community resource knowledge.

Maintain compliance with regulatory requirements and ensure accurate data collection for case management.

Qualifications:
  • Education: Bachelor's degree in Nursing from an accredited institution or a Master's degree in Nurse Case Management.
  • Experience: Minimum of 5 years as a Registered Nurse Case Manager within the last 7 years.
  • Certification: Current certification from an accredited organization in case management.
  • Licensure: Active and unrestricted Registered Nurse license.
  • Additional Requirements: Ability to pass a government background check and possess current BLS certification.
Compensation:

Hourly rate of $65.00, commensurate with qualifications and experience.
