Job Summary:
As a Community Health Worker at LifeBridge Health, you will play a vital role in providing direct services to individuals and families in need. Your primary responsibility will be to extend psychosocial support services by means of education, advocacy, and services coordination.
Key Responsibilities:
- Gather information related to individual and family, financial, health, and emotional status to develop a plan for addressing psychosocial barriers to health and well-being.
- Provide education and psychosocial support on issues such as health, medical compliance, budgeting, safety, parenting, and community resources according to assessed need and resulting plan.
- Develop and implement client-directed Service Plans to address psychosocial barriers to health and well-being.
Requirements:
- Formal working knowledge equivalent to an Associate's degree (2 years college) in a specialized field.
- 1-3 years of experience in a related field.
- Driver's License and personal transportation preferred and may be required for some service areas.
