Job Description:We are seeking an experienced MDS Coordinator (RN License Required) to join our team. In this role, you will be responsible for maintaining and following schedules for MDS assessments, coordinating completion of MDS by all disciplines, and reviewing data for accuracy.
- Maintain and follow schedules for MDS assessments
- Coordinate completion of MDS by all disciplines
- Review MDS data for accuracy and meet with staff as needed
- Complete MDS and input into computer system
- Verify timely transmission and approval of assessments by State
- Correct rejected records and prepare for re-submission
- Complete CAAs according to state guidelines
- Develop Resident Care Plans in collaboration with interdisciplinary team
- Schedule and conduct RCP meetings on regular basis
- Communicate RCP to staff members and notify supervisor of non-compliance
- Registered Nurse with current, valid RN license from North Carolina Board of Nursing
- Ability to read, know, and follow policies and procedures
- Experience with MDS-RAP and Care Planning functions
- Preference for experience with RUG-IV and MDS 3.0
