**Care Planning and Coordination Role:**

We are currently looking for a highly skilled RN Licensed Healthcare Manager to join our team at Liberty Health. As an MDS Coordinator, you will be responsible for maintaining and following a schedule of due dates for all MDS assessments.

Key Responsibilities:

  • Coordinate the completion of MDS by all disciplines.
  • Review MDS data for accuracy and meet with staff as needed to ensure accuracy.
  • Complete MDS and input into the computer system.
  • Verify timely transmission and approval of assessments by the State.
  • Correct rejected records and prepare them for re-submission.
  • Develop Resident Care Plans (RCP) in collaboration with the interdisciplinary care team.
  • Schedule and conduct RCP meetings on a regular basis.

Requirements:

  • Registered Nurse degree from an accredited School of Nursing and a current, valid RN license from the North Carolina Board of Nursing.
  • Able to read, understand, and follow personnel, department, and facility policies and procedures.
  • Experience with MDS-RAP and Care Planning functions.
  • Prefer experience with RUG-IV and MDS 3.0.

Estimated Salary Range: $65,000 - $80,000 per annum, depending on experience.
