About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Humana Inc. As a Home Health Registered Nurse, you will play a vital role in delivering high-quality patient care and improving health outcomes in the comfort of patients' homes.

Key Responsibilities
  • Provide comprehensive nursing care to patients with complex medical needs, including admission, case management, and follow-up visits.
  • Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans that address patients' physical, emotional, and social needs.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans that promote optimal health and well-being.
  • Coordinate care with other healthcare disciplines, including physical therapy, occupational therapy, and home health aides, to ensure seamless and comprehensive care.
  • Document patient care and progress accurately and thoroughly, ensuring compliance with regulatory requirements and company policies.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum of one year of nursing experience, preferably in home health or a related setting
  • Strong medical-surgical, ICU, ER, and acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation
