RN - Disease Management Specialist

Spectrum Healthcare Resources is seeking a highly skilled Registered Nurse Disease Manager to join our team. As a Disease Management RN, you will be responsible for performing assessments, education, and training to coordinate care for the dependents of our Nation's Active Duty and eligible retirees.
The ideal candidate will have a Bachelor's Nursing Degree, at least one active state nurse license, and at least 2 years' experience working as an RNDM. Additionally, they should possess current certification in disease-management related areas such as Chronic Care Professional (CCP), Certified Health Coach (CHC), Certified Diabetes Educator (CDE), or Certified Asthma Educator (AE-C).
The successful candidate will be highly organized, self-directed, and able to function in a high-volume environment without distractions. They will also have strong verbal and written communication skills, proficiency in Microsoft Office applications, and a strong technical aptitude.
