About the Role:
Spectrum Healthcare Resources is seeking a highly skilled Registered Nurse Disease Manager to join our team. As a Remote Registered Nurse Disease Manager, you will be responsible for performing assessments, education, and training to coordinate care for the dependents of our Nation's Active Duty and eligible retirees.
Key Responsibilities:
- Perform assessments and develop care plans to ensure optimal health outcomes for patients
- Provide education and training to patients and their families on disease management and self-care
- Coordinate care with healthcare providers and other stakeholders to ensure seamless transitions
- Utilize strong communication and interpersonal skills to build relationships with patients, families, and healthcare teams
Requirements:
- Bachelor's degree in Nursing
- Active state nurse license, preferably compact state licensure
- At least 2 years' experience working as an RNDM, preferably in a remote or hybrid environment
- Certification in disease-management related area, such as CCP, CHC, CDE, or AE-C
