Home Health Care Registered Nurse Full-time Position

VNS Health is seeking a skilled Registered Nurse (RN) to join our Home Health team in New York. As a member of our team, you will have the opportunity to work with patients in their homes, providing personalized care and support.
Job Summary:We are looking for a dedicated and compassionate RN to join our Home Health team. The successful candidate will have a strong passion for delivering high-quality patient care and a commitment to excellence in their work.
Responsibilities:- Deliver personalized nursing care to patients in their homes, including assessments, care planning, and evaluation of care.
- Participate in classroom learning activities, including topics such as critical thinking, evidence-based best practice assessments, care planning, and evaluation of care.
- Shadow experienced RNs to gain field experience and participate in group debriefs to review experiences.
- Practice in the community with a preceptor as part of an interdisciplinary care team.
- Constantly evaluate evolving patient needs and respond with plan of care adjustments.
- Achieve continued professional growth through attendance at in-services, training, continued formal education, and select journal readership.
- Recently received RN license.
- Degree in Nursing from an approved program required.
- No prior work experience in nursing required.
- Must be able to walk up and down steps to access patient residences.
