About the Role:

The University of California is seeking a dedicated and compassionate Registered Nurse to fill the position of Homeless Health Advocate. As a key member of our team, you will provide critical care and support services to vulnerable populations in need.

Key Responsibilities:

  1. Develop and implement effective care plans in collaboration with interdisciplinary teams.
  2. Provide direct patient care and conduct comprehensive assessments to identify individual needs.
  3. Cultivate partnerships with local organizations to enhance access to resources and support services.
  4. Operate the Community Care Van to facilitate outreach and engagement efforts.

Essential Qualifications:

  • California RN licensure and active membership in relevant professional associations.
  • Strong background in case management, social work, or related field.
  • Familiarity with community-based healthcare models and best practices.
  • Valid California driver's license and ability to drive company vehicles.
  • Excellent communication and interpersonal skills.

Compensation Package: Up to $88.39 per hour
