Responsibilities:

As a LPN Homecare Specialist, your primary responsibilities will include:

  • Utilizing the nursing process to assess, plan, implement, and evaluate patient care
  • Providing skilled nursing care and services in accordance with prescribed orders
  • Assessing signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
  • Collecting, analyzing, and interpreting data and information from healthcare members and documenting actual and/or potential nursing diagnoses
Qualifications:

To be considered for this role, you must:

  • Be currently licensed as an LPN/LVN in the state in which you will practice
  • Have a current TB or Chest X-Ray
  • Hold a current BLS card
  • Have at least one year of prior professional nursing experience
