Job Summary:
Signature Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Patient Care Specialist. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to our patients and families.
Key Responsibilities:
- Assist with Patient Care: Escort patients from the waiting area to the examination room, prepare patients for visits, and obtain vital signs as needed.
- Document Patient Information: Accurately document patient information in the electronic medical record, including reason for visit and patient acuity.
- Assist with Medical Procedures: Assist physicians and licensed providers with medical and surgical procedures, including obtaining and operating medical equipment as needed.
- Provide Patient Education: Educate patients on care and prevention of illness and injury, within the scope of standards of care of nursing practice.
- Maintain a Clean and Safe Environment: Clean, prepare, and stock examination rooms and treatment areas, and assist with the care, cleaning, and maintenance of medical equipment and instruments.
- Communicate Effectively: Communicate effectively with patients, families, and healthcare team members, including physicians, licensed providers, and other staff.
- Support Patient-Centered Care: Support the Patient-Centered Medical Home model, promoting primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective.
Requirements:
- Education: Graduate of an approved practical nursing program.
- Experience: One-two years of successful working experience as a Licensed Practical Nurse preferred.
- Certification/Licensure: Licensed in the Commonwealth of Massachusetts as a Licensed Practical Nurse. Current American Heart Association BLS for Healthcare Providers completion.
- Software/Hardware: Meditech experience preferred.
