Job Title: Registered Nurse - Geriatrics

Element Care is seeking a skilled Registered Nurse to join our team as a Geriatric Nurse Specialist. In this role, you will be responsible for providing high-quality nursing care to our elderly clients, working collaboratively with our interdisciplinary team to develop and implement comprehensive care plans.

Key Responsibilities:
  • Provide direct care to participants, including assessments, care planning, and implementation of care plans.
  • Collect and analyze medical and social information to ensure comprehensive decisions are made regarding participant care.
  • Monitor clinic care and service delivery to ensure conformance with established quality assurance and operational policies, procedures, and standards.
  • Remain attentive to participants' health status and social functioning, identifying and addressing problems and needs as they arise.
  • Coordinate with the interdisciplinary team to develop and implement comprehensive care plans for each participant.
  • Actively participate in implementing a summary on each client to help the state determine Nursing Home Level of Care.
  • Ensure all tests, lab work, and diagnostic studies ordered by the physician have been carried out.
  • Coordinate all medication, including stocking, ordering, administering, and monitoring participant compliance.
  • Provide education and consultation to participants and caregivers regarding care plans, nursing treatments, and medications.
  • Communicate participants' conditions to primary caregivers, physicians, and the multidisciplinary team.
  • Build strong relationships with participants, caregivers, families, and community providers to facilitate optimal care and financial oversight.
  • Participate in after-hour visits as needed.
Requirements:
  • Registered Nurse currently licensed in the Commonwealth of MA.
  • Minimum of two (2) years experience with the geriatric population.
  • Proficient in the use of Microsoft Office tools.
  • Ability to travel locally on a frequent basis in a personal vehicle.
  • Ability to quickly recognize situations/problems before they become acute and implement effective solutions.
