KEY RESPONSIBILITIES:


Manage the MDS process in accordance with the RAI manual. Collaborate with other MDS nurses to perform initial assessments for new admissions.

Work under the supervision of the Director of Nursing to ensure MDS nurses effectively handle the scheduling of patient admissions, including comprehensive assessments, order entries, initiating care plans, and ensuring that patients have the required equipment upon admission. Complete MDS assessments and care planning as needed, and submit required documentation weekly or as directed by the Director of Nursing.

During submissions, review validation reports to ensure accuracy and compliance. Maintain the ability to obtain and assess Quality Measures (QM's).

Collaborate with the Interdisciplinary Team (IDT) to initiate quality improvement projects aimed at enhancing resident outcomes. Participate in IDT Medicare and Medicaid meetings to work with the team in completing the MDS process.

Develop and update comprehensive care plans and nurse aide care plans (Kardex) in conjunction with the MDS to accurately reflect resident needs. Ensure that non-nursing sections of the MDS and Care Area Assessments (CAA's) accurately represent resident requirements.

Coordinate, manage, and monitor care plans, identifying resident problems and needs, and reviewing order recap reports and nursing notes to ensure care plans are followed. Assess residents for changes in status to determine the need for significant changes per RAI guidelines. Ensure that all documentation is accurate and compliant with MDS/RAI regulations.

Collaborate with facility consultants as necessary and implement recommended changes as required. Complete necessary paperwork including AIMS, Fall Risk assessments, Braden scales, progress notes, care plans, and nurse aide care plans (Kardex).

QUALIFICATIONS: Must possess a current RN license for the state in which the facility operates. Flexibility in scheduling and the ability to work independently are essential. A solid understanding of the MDS process and a commitment to ongoing learning and staying updated with MDS changes set by CMS are required. Knowledge of PDPM (skilled reimbursement) and RUG (Long Term Care reimbursement) is preferred.
