About the Role

We are seeking a highly skilled Admissions/MDS Nurse to join our team at Carespring in Cincinnati, OH. As a key member of our healthcare team, you will play a vital role in ensuring the highest level of care for our residents.

Key Responsibilities
  • Oversee the MDS process, adhering to the RAI manual and ensuring accuracy in documentation.
  • Collaborate with other MDS nurses to complete initial assessments for new admissions, ensuring a seamless transition for our residents.
  • Manage patient admissions scheduling, MDS assessment, and care planning, ensuring timely and effective care.
  • Submit required reports weekly or as directed by the DON, maintaining the highest level of accuracy and attention to detail.
  • Review validation reports to ensure error-free submissions, upholding the highest standards of quality care.
  • Obtain and review Quality Metrics (QM's) to inform care decisions and improve resident outcomes.
  • Work closely with the Interdisciplinary Team (IDT) to develop and implement care plans that promote resident well-being and quality of life.
  • Attend IDT meetings to discuss Medicare and Medicaid matters, ensuring compliance with regulatory requirements.
  • Develop and update comprehensive care plans, addressing the unique needs and preferences of each resident.
  • Ensure non-nursing sections of the MDS and Care Area Assessments (CAA's) accurately reflect resident needs, promoting a holistic approach to care.
  • Monitor care plans to identify resident needs and make adjustments as necessary, ensuring the highest level of care and quality of life.
  • Review order recap reports and nurses' notes to ensure care plans are followed and determine the need for additional OBRA MDS assessments.
  • Monitor residents' changes in status and determine the need for changes per RAI, ensuring timely and effective care.
  • Ensure accuracy of supportive documentation, adhering to MDS/RAI regulations and promoting a culture of compliance.
  • Collaborate with facility consultants to implement recommended changes, ensuring continuous quality improvement.
  • Complete necessary paperwork, including AIMS, Fall Risk scale, Braden, Progress note, Care Plan, and nurse aide care plan (Kardex) documentation.
Requirements
  • A current LPN or RN license in the state of Ohio.
  • A flexible work schedule and the ability to work independently, with a strong attention to detail and organizational skills.
  • An understanding of the MDS process and a willingness to continually learn and grow in this role.
  • Knowledge of PDPM and RUG, with the ability to apply this knowledge in a practical and effective manner.
