As a Community Health Nurse, you will provide health promotion and disease prevention services, community health nursing services, and home health services based on current community health nursing concepts and standards of practice.

Key Responsibilities:

  • Provide direct patient care and document services within assigned workload.
  • Demonstrate accountability and problem-solving skills.
  • Incorporate professional standards of practice and maintain accurate records.
  • Develop, implement, and update individualized nursing care plans for clients.
  • Teach and instruct patients and families in positive health measures.
  • Intervene to promote, maintain, or restore client health and prevent illness.
  • Provide primary health care and generalized public health nursing services.
  • Utilize the nursing process to assess, plan, implement, and evaluate health needs.
  • Participate in peer reviews to ensure quality nursing care and best practices.
  • Collect and analyze client and community data to determine diagnoses and plan care.
  • Enforce compliance with community health nurse standards of practice and accreditation standards.

Requirements:

  • Associate Degree in Nursing required; Bachelor's degree in nursing preferred.
  • Minimum of 1 year of experience as a Registered Nurse required.
  • Current R.N. licensure in the State of Michigan required.
  • CPR Certification and valid driver's license required.
  • Ability to establish and maintain effective communication with co-workers, supervisors, subordinates, and the general public.
  • Knowledge of nursing theory, skills, and best practice standards required.
  • Knowledge of prevention, transmission, and treatment of disease required.
  • Knowledge of infection control, safety regulations, and standards of practice for community health nursing and ambulatory care required.

Working Conditions:

The Community Health Nurse position requires frequent exposure to weather, extreme heat/cold, wet/humidity, noise, and occasional exposure to air quality. Personal protective equipment is provided, including gloves for CO monitor testing.
