Position Title: Project Case Manager

SUMMARY:

This role is pivotal in supporting individuals with disabilities. Reporting directly to the Project Director, the Project Case Manager is tasked with overseeing all responsibilities related to the assigned caseload.

KEY RESPONSIBILITIES:

  1. Facilitates the completion of client intake documentation and assessments.
  2. Secures necessary medical and psychological evaluations.
  3. Contributes to the development of Individual Service Plans (ISP).
  4. Assists in formulating skills and behavioral programs for clients while collecting relevant data.
  5. Imparts skills to clients based on the ISP and diligently records their progress.
  6. Maintains regular communication with clients' families.
  7. Applies for Supplemental Security Income (SSI) and Social Security (SS) benefits, ensuring proper documentation is filed.
  8. Prepares monthly reports and manages other assigned administrative tasks.
  9. Oversees medication logs and self-administration of medication, when applicable.
  10. Serves as an on-call case manager as scheduled, which may include operating a lift to assist non-ambulatory clients with daily living activities.
  11. Provides essential services to clients, including:
  • Transportation (may involve operating a lift-equipped vehicle)
  • Scheduling medical appointments
  • Offering counseling services
  • Providing information and referrals
  • Assisting with shopping
  • Guiding budgeting and money management
Completes training on the Regional MIS System, ensuring timely entry of clients' basic information, treatment plans, and progress notes.Advocates for and protects clients' rights, particularly their ability to make informed choices regarding their care and daily living, as long as these choices do not compromise their wellbeing.Performs additional duties as assigned.

QUALIFICATIONS:

Required: A Bachelor's degree in Sociology or a related field, along with experience working with individuals with disabilities. The candidate must be capable of bending, lifting (up to 40 pounds), and assisting individuals with transfers. Excellent customer service skills are essential.
