Job Title: Home Infusion Nurse

We are seeking a skilled and compassionate Home Infusion Nurse to join our team at Naven Health. As a Home Infusion Nurse, you will play a critical role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes.

Job Summary:

The Home Infusion Nurse is responsible for coordinating and providing direct infusion care to patients in their homes or alternate treatment sites. This includes assessing patient needs, developing and implementing individualized plans of care, and ensuring continuity of care in compliance with physician orders.

Key Responsibilities:
  • Plan, provide, and document professional nursing care for patients in accordance with physician orders and established policies and procedures.
  • Use professional judgment to develop and individualize plans of care and physician orders based on patient assessment and response to care.
  • Perform treatments, administer medications, and educate patients and families/caregivers.
  • Assess patient needs and physical status each skilled visit through health data assessment and patient interview.
  • Re-evaluate patient needs through physical assessment, response to therapy, and updated physician orders.
  • Communicate effectively with patients, caregivers, healthcare teams, physicians, and administrators to ensure continuity of care.
  • Complete clinical documentation in real-time during nursing visits, following nursing standards of practice and applying approved abbreviations and documentation practices.
  • Ensure notes are transmitted promptly to comply with established timelines for billing optimization.
  • Coordinate discharge planning, including preparing discharge summaries with patient instructions.
Requirements:
  • Graduate of an accredited School of Nursing.
  • Current active and unrestricted registered nurse license.
  • At least 2 years of clinical experience working as a RN in an area requiring training and hands-on experience in infusion therapies.
  • Current certification in Basic Life Support for Health Care Professionals.
  • Valid Drivers License and proof of liability insurance with access to a vehicle for business travel.
  • Basic knowledge of computer operating systems and software applications with the ability to apply knowledge and assimilate to the various nursing technology tools utilized by Naven Health.
Preferred Qualifications:
  • Bachelor of Science in Nursing from an accredited school of nursing.
  • CRNI, IgCN, OCN highly preferred.
